The Sample Company provides comprehensive benefits for eye examinations and corrective lenses through Guardian. The vision plan is built around a network of eye care providers, with better benefits at a lower cost to you when you use providers who belong to the VSP network. When you use an out-of-network provider, you will have to pay more for vision services.

Eye exams are a great way to keep tabs on what is happening in your body. Your eyes are the only place in your body that provide a clear view of your blood vessels, arteries, and a cranial nerve, which can tell your doctor a lot about your overall health.

How to find an in-network provider:

You may locate participating vision providers by accessing the Guardian website at www.guardiananytime.com.

• Select Find a Provider in the part of the screen.
• On the Select Your Vision Plan list, choose “VSP.”
• Once you have completed the other requested information, click “Continue.”

You can print an ID card by registering as a member at www.guardiananytime.com, cards are not sent to members at their home address.
